Diabetes Management
Managing Diabetes: A Comprehensive Approach
Diabetes is a complex chronic condition that typically requires lifelong management. It involves daily self-care through diet, medication, and general health practices. Our goal is to support patients in effectively managing their diabetes and maintaining a high quality of life.
Types of Diabetes:
- Type 1 Diabetes: An autoimmune condition where the body does not produce insulin.
- Type 2 Diabetes: A condition where the body does not use insulin effectively, often linked to lifestyle factors.
- Gestational Diabetes: Occurs during pregnancy and usually resolves after childbirth but increases the risk of developing type 2 diabetes later in life.

Our Approach to Diabetes Management:
Our Doctors and Nurses work closely with patients to develop personalized goals and care plans. We emphasize a collaborative approach to ensure that each patient’s unique needs are addressed.
Available Resources:
- Diabetes Educators: Provide essential education and support for managing diabetes.
- Dieticians: Offer guidance on dietary changes and nutrition to manage blood sugar levels.
- Podiatrists: Address foot care and complications related to diabetes.
- Exercise Physiologists: Develop exercise plans to improve overall fitness and manage weight.
- Physiotherapists: Assist with physical therapy to enhance mobility and strength.
Diabetes Reviews:
Our practice encourages diabetic patients to have regular reviews every 3-6 months, depending on their control, in addition to an annual review. These reviews include:
- Blood Tests and Monitoring: Regular assessment of blood glucose levels and other relevant parameters.
- Annual Diabetes Review: Comprehensive evaluation of diabetes management and health status.
Referrals and Coordination:
During these reviews, referrals to Allied Health Providers, such as Podiatrists and Diabetes Educators, can be arranged to ensure comprehensive care and support.
Our integrated approach ensures that every aspect of diabetes management is covered, helping patients achieve better control and overall health. For any questions or to schedule a review, please contact our practice.
