We offer a Healthy Heart Check to proactively screen adult patients who may be at risk of developing heart conditions. Early detection is key to preventing serious cardiovascular issues. For those who have experienced a cardiac event or are at risk, our Cardiac Rehabilitation Program provides essential support for recovery and improved heart health.

When a patient is identified as high risk or is recovering from a cardiac event, they will be scheduled to see our dedicated Nurse. This Nurse works closely with the patient’s Doctor to develop an individualized plan of action.

Our Cardiac Rehabilitation Program includes:
- Collaborative Care: The Nurse and Doctor work together to create a personalized care plan.
